Our Lifeline Home Care program is designed to meet the needs of older adults and people with disabilities who wish to live as independently as possible for as long as possible in their home. Our program of in-home support starts with a comprehensive assessment and an individualized care plan.

Comprehensive Needs Assessment
Not only are our JFCS staff professionally trained and experienced in geriatrics, they are caring respectful and supportive as they work with you and your family to assess your individual needs. We are also there when your needs change to ensure that you have the proper supports you need to remain independent in your home.

Individualized Care Plan
We can create a unique plan for each senior that works in unison with family members, caregivers, physicians, mental health providers, and others involved in the older adult’s support system.

 

JFCS Home Care Services
We provide seniors and people with disabilities assistance in performing daily tasks around the house that include:

Light Housekeeping and Special Tasks
Our Lifeline specialists assist with laundry, cleaning the refrigerator, dusting, and taking out the garbage.

Errands and Shopping
Engage a Lifeline specialist to assist in running errands for you such as picking up dry cleaning and prescriptions, shopping for groceries, household supplies, personal items, a special birthday or holiday gift, or flowers to brighten a room.  If you desire, you have the option going with the specialist to run your errands, or you may send them on their own.

Meal Preparation and Menu Planning
Plan a special meal, or simply ensure that you are eating well-balanced, healthy meals with the help of one of our Lifeline specialists.  We can prepare meals in advance or on a daily basis. The Lifeline specialists can also help you plan your menus before your next shopping day.

Transportation
Lifeline specialists can arrange for transportation to and from medical appointments, social engagements, shopping and errands or visits to all of your desired locations.

Companionship
A lifeline companion can provide the personal connection and interaction that all of us need and which is vital for living healthy, productive lives.

Bill Paying
Leave the frustration of managing, paying and coordinating bill payments to a specialist at Lifeline. Not only can it simplify your life but it can also help seniors who are facing vision loss or other physical problems to read the fine print and keep vital services like utilities and telephones in working order.
