The comprehensive JFCS Lifeline program is designed to meet the needs of seniors and people with disabilities who wish to live as independently as possible for as long as possible in their home.
We offer a variety of services for individuals and their families.
- Professional, in-home assessments of emotional, physical and social needs
- Home care assistance: companionship, meal preparation, light housekeeping, and transportation.
- Individual and group counseling. Fees may be reimbursable by Medicare and private insurance.
- Care management

- Coordination of community services

- Holocaust survivor services and support
Counseling
Lifeline family counseling services involve identifying and assessing individual and family needs including issues related to transitions in aging. The counseling includes identifying how individual family members feel about the changes that are happening to their family as a result of the aging process in the older adult. Lifeline individual counseling done directly with seniors, involves providing the older adult with the opportunity to talk about their life circumstances, feelings and any other aspect of their life where they would like support or guidance. Care Management
Assessments are completed by our experienced and professional staff in your home, at a care facility or in the office to identify the issues that are most important to you and your family member. These may include social supports, family history, geriatric issues, daily activities, safety concerns, mental health issues and anything that may influence the security of a senior’s current living situation.
Home Care
Our Lifeline Home Care provides seniors and people with disabilities assistance in performing daily tasks around the house that include: housekeeping, errands and shopping, meal preparation & menu planning, transportation, companionship and bill paying. To learn more.
Consultation
JFCS Consultation involves providing information to individuals and families about resources, the processes involved in accessing these resources and any information they may need to make decisions about their elderly family member’s circumstances. In addition, the JFCS staff can discuss specific information with extended family members or professionals who are working with the senior and his/her family.
