Providing 1:1 Skills Training
to individuals
served by
Support Service Brokerages
in the Portland Metro Area
Individuals being served in our program will receive supports in a holistic manner whenever appropriate, to include their case management services, their other goals and services, and their surrounding communities. We work towards meeting customer goals in an effective manner, with respect for efficient Plan spending. We believe that high-quality supports include meaningful communication and solid documentation of supports and progress towards goals.
Our agency offers the following benefits to customers:
- An extensive network of supports and an internal resource database
- Staff with extensive experience working in Support Services, both as a provider and within the Brokerages
- A program that encourages long-term employees, leading to a greater continuity of support for customers
We at Partners feel that a person’s life is enriched by learning skills that can be used to increase independent living, create or expand a fulfilling social network, and draw out talents and interests, helping people to achieve their highest quality
of life.
Partners staff approach each customer with respect to their individuality, and carefully structure a plan of approach designed to assist them towards their chosen goals.
Skills Trainers will work with the customer to identify benchmarks within the goal against which to gauge progress. Through one-to-one supports the Skills Trainer will identify assets and analyze barriers to the customer’s success. We aim to assist individuals to learn effective solutions as well as new skills.
The Partners program offers many services, some of which include:
- Social and community involvement
