Important Steps to Complete Before We Can Schedule Your Appointment
Welcome (or welcome back!) to JFCS. Before we can schedule your appointment with a JFCS provider, we need you to complete a few required steps in our secure client portal.
1. Watch for Your Apricot (Social Solutions) Portal Invitation
You will soon receive an email from an address containing the words “social solutions.”
Please check your spam or junk folder if you don’t see it in your inbox.
2. Set Up Your Portal Account
When you open the invitation:
- Use your email address as your username.
- Create a password.
- Log in to the portal to access your documents.
3. Complete the Required Documents
In the portal, you will see several documents.
Any document marked with (★ Required) must be completed before therapy can begin.
Please complete these required items:
- Insurance Information (★ Required)
- Upload copies of your insurance card(s) and include all policy information.
- Adult Personal History Form (★ Required)
- Signature required. Please share only what you feel comfortable sharing.
- Notice of Privacy Practices (★ Required)
- Signature required at the end of the notice.
- Clinical Payment Policy (★ Required)
- Signature required.
- Required Consent for Treatment, Orientation & Services Information (★ Required)
- This is your formal consent to receive services.
4. Tip: One Form Is Lower on the List
The Consent for Treatment form may appear farther down the document list.
You may need to scroll or use the right‑side arrow to reach it.
This process shouldn’t take too long, and completing these forms ahead of time allows us to schedule your appointment as quickly as possible. We appreciate your cooperation! If you need help, just reach out — we’re here for you.
